About Continuing Healthcare

The CHC Network is a group of experienced senior care professionals driving improvements in the CHC process across Yorkshire and Humber.

Chair: Andrew Wheawall

The region’s CHC network is made up of continuing healthcare leads from the 15 Y&H local authorities.  Regular meetings are held to oversee and understand:

  •  the NHS CHC activity related to the COVID-19 emergency period, and any assessments that have been deferred as a result of the COVID-19 Hospital Discharge Service
  • Support local health and social care systems to jointly plan their workforce to manage the NHS CHC deferred assessment work and agree a joint workforce and recovery plan
  • Collate and share the learning from Independent Review Panels
  • Participate in learning events and workshops
